This is the bare PCB that is used in the Skyworks-based LNA that's on my Tindie store. It is compatible with the 67012, 67013, 67014 and 67015 chips.

These support the frequency ranges of:

300MHz-600MHz
600MHz-1.5GHz
1.5GHz-3.0GHz
100MHz-300MHz respectively.
